Smile design is not a single treatment — it is a personalized transformation process. The appearance of the gums, lip form, facial symmetry and jaw structure are just as integral to this process as the color, length and form of the teeth. Aesthetic applications performed without proper planning may yield short-term results but will not be natural or lasting in the long term. As Sevgi Savaş Dental Clinic in Ankara İncek, we offer every patient a comprehensive analysis first, followed by a personalized treatment plan.

The Aesthetic Smile Design Process

Smile design is a planning process far more comprehensive than a few sessions spent at the clinic. Each stage directly affects the accuracy of the next treatment. An aesthetic restoration applied without correct diagnosis and planning can lose its function in a short time.

1
Comprehensive Clinical and Radiological Examination

Gum health, presence of decay, bite pattern (occlusion) and the temporomandibular joint are evaluated. Aesthetic planning begins on a healthy foundation.

2
Photograph and Video Analysis

Detailed photographs are taken in smiling, resting and speaking positions. Facial contours, tooth-lip ratios and midline analysis are performed.

3
Visagism and Golden Ratio Assessment

The most appropriate tooth form is determined according to visagism principles based on the person's facial geometry, skin tone and lip structure.

4
Digital Smile Design (DSD)

A digital design is created with specialist software. The patient sees their potential smile on screen without any procedure having yet been performed.

5
Mock-Up (In-Mouth Prototype)

The digital design is simulated directly in the mouth with temporary composite or silicone material. The patient "tries on" the design and approves it.

6
Preparatory Treatments (If Required)

Preparatory procedures such as ensuring gum health, pink aesthetics or orthodontic correction are completed.

7
Final Restoration and Placement

The approved design is produced in the laboratory, passes through clinical checks and is placed in the mouth. Occlusion check is performed.

Visagism: Matching the Smile to the Personality

Visagism is a special approach used in aesthetic dentistry that determines the tooth form according to scientific principles by taking into account a person's facial contours, character, skin tone and personal style. It rejects the idea of "the same teeth for everyone" and instead creates an aesthetic language unique to that individual.

Our clinic in Ankara İncek is one of the pioneering centers applying the visagism method. This method provides a level of personalization that common competing clinics do not offer.

Round Facial Features

Softer, oval and rounded tooth forms are preferred. Angular forms emphasize the roundness of the face and are therefore avoided.

Oval / Elliptical Face

Compatible with many tooth forms. Slightly angular and balanced forms may be preferred; the most flexible face type.

Square / Sharp Facial Features

More angular and wider tooth forms achieve aesthetic harmony; they create a rhythm with the facial structure.

Triangular Facial Features

Narrower and longer forms are preferred. The appearance of the upper front teeth is carefully balanced to achieve proportion.

Digital Smile Design (DSD)

Digital Smile Design (DSD) is a modern planning method that combines photographic and video analyses with specialist software to visualize the final outcome before treatment begins.

What Is Possible with DSD? Digital smile simulation before treatment · Face-tooth ratio and midline analysis · Precise planning of tooth length and width ratios · Full alignment of expectations between patient and dentist · Digital design file facilitating laboratory communication
Especially Recommended For? Patients planning a comprehensive smile transformation (Hollywood smile) · Cases requiring a combination of multiple treatment methods · All patients wishing to obtain visual approval of the outcome · Treatments planned with orthodontics + veneer combination

Mock-Up: Try Your Smile Before It's Permanent

Mock-up is the stage at which the digital design is simulated directly in the mouth using temporary composite material or a silicone guide. Without any permanent procedure having yet been performed, the patient sees and feels the planned smile in their mouth and can show it to friends or family members.

This stage is one of the most valuable steps in smile design. The patient expresses any detail they do not approve at this point; the dentist then applies the necessary revision before proceeding to the permanent restoration. This ensures both patient satisfaction and the durability of the treatment.

What Questions Does Mock-Up Answer? How long or wide will the teeth be? · Is the midline correct? · How will tooth length affect my speech and smile? · Does this color harmonize with my skin tone? · Does the gum line look symmetrical?

Porcelain Laminate Veneer (Leaf Tooth)

Porcelain laminate veneer is the most frequently used restoration method in smile design. Thin porcelain layers 0.3–0.7 mm thick are bonded to the front surfaces of the teeth, resolving color, form and dimension problems simultaneously. It can be applied with minimal tooth preparation or, with the "non-prep" version, with no preparation at all.

For front teeth, porcelain laminate veneer produces results virtually indistinguishable from natural teeth thanks to its natural light transmittance.

Zirconia Restorations

Zirconia is a metal-free, biocompatible and extremely durable dental ceramic. In smile design it is preferred in comprehensive cases covering the back teeth as well, or in patients where more tooth tissue has been lost in the front teeth. Its property of not causing dark discoloration at the gum margin is a critical advantage for long-term aesthetics.

When applied in combination with porcelain laminate veneer — veneer in the front region, zirconia in the back — the most comprehensive result is achieved both aesthetically and functionally. The balanced use of both materials in smile design planning is a direct reflection of the clinician's experience and laboratory quality.

Pink Aesthetics and Gum Arrangements

Even the most perfect veneer application cannot achieve the desired aesthetics when combined with asymmetric or excessively visible gums. Pink aesthetics is the invisible but indispensable component of smile design.

  • Gummy smile (excess gum): Excessive visibility of gums when smiling is corrected by laser gingivectomy or surgical methods.
  • Asymmetric gum levels: Gum heights above different teeth are equalized to create a symmetrical line.
  • Crown lengthening: The visible length of teeth that appear short or are too deeply embedded in the gum is extended; this may be necessary for both aesthetic and veneer placement purposes.
  • Gum lightening: In dark discolorations due to melanin pigmentation, lightening can be applied with laser.

The need for pink aesthetics is always evaluated when smile design planning is being done; it is completed before veneers when required.

Teeth Whitening (Bleaching)

Teeth whitening is the fastest and least invasive component of smile design. When applied alone, it provides a significant aesthetic improvement; when applied together with porcelain veneer or zirconia restoration, it transforms the entire smile line as a whole.

The in-office bleaching applied at our clinic is performed with high-concentration whitening gel under gum protection. Lightening of 2–6 shades in a single session may be possible. If veneer or crown is planned, whitening is performed first at the shade selection stage; the restoration color is selected according to this reference shade.

Orthodontic Support: Solid Foundation, Lasting Aesthetics

One of the most frequently overlooked steps in smile design is orthodontic preparation. Applying veneers to crowded or incorrectly positioned teeth both increases tooth preparation and shortens restoration longevity in the long term. First aligning the teeth correctly and then performing aesthetic applications is always a more predictable and tissue-preserving approach.

Clear Aligner Invisible, removable and comfortable · 4–18 months for mild-moderate crowding · Ideal choice for pre-veneer preparation · Preferred aesthetic orthodontics for adult patients
Bracket Systems Porcelain (aesthetic) or metal options · For complex bite disorders · In cases where clear aligner is insufficient · Situations requiring faster tooth movement

Smile Design – Frequently Asked Questions

What is smile design?
Smile design is a set of personalized aesthetic treatments aimed at harmonizing the color, form, length and alignment of teeth and the appearance of the gums with lip form, facial symmetry and jaw structure as a whole. It is not a single procedure; it is a multidisciplinary process covering visagism analysis, digital planning, mock-up approval and appropriate restoration methods.
How many days does smile design take to complete?
Treatments involving only porcelain laminate veneer or teeth whitening can be completed within 1–3 weeks. Cases requiring pink aesthetics preparation take a little longer. Comprehensive plans involving orthodontic preparation can range from a few months to 1.5 years. At our clinic, the digital planning and mock-up stage makes the timeline predictable and is shared with the patient from the outset.
Is smile design permanent?
It varies depending on the method used: porcelain laminate veneer and zirconia restorations last 10–20 years with correct care, and composite bonding lasts 5–7 years. Orthodontic treatment results are made permanent with retainer use. Regular dental check-ups and good oral hygiene directly extend the lifespan of all restorations.
What is a mock-up and why is it important?
Mock-up is the stage at which the planned smile is simulated in the mouth with temporary material. Without any permanent procedure, the patient sees the length, form and color of their new teeth in their mouth. This way, every detail passes through patient approval before treatment begins; unexpected surprises and revisions are minimized. It is the strongest guarantee of quality.
What is the difference between visagism and standard smile design?
In the standard approach, teeth are shaped with aesthetic proportions, but the person's facial structure is not always taken sufficiently into account. Visagism, however, determines a tooth form unique to that individual by analyzing facial geometry, skin tone, eye shape and personal character together. The result is a smile that looks not artificial but "becoming" to that person. The number of centers applying this method in Ankara is quite limited.
Is smile design painful?
Veneer preparation and crown procedures are performed under local anesthesia; no pain is felt during the procedure. Mild sensitivity may be experienced for a few days during the temporary restoration stage. Laser gum aesthetics applications are also extremely comfortable. Teeth whitening and mock-up stages do not require anesthesia.
What should be done before smile design?
First of all, gum inflammations, tartar and cavities must be treated. If there are teeth requiring root canal treatment, these are completed first. Orthodontic preparation is planned in cases where it is necessary. Aesthetic restorations made without establishing a healthy foundation are not long-lasting; therefore the preparation stage is an inseparable part of aesthetic treatment.
How is the choice made between porcelain laminate veneer and zirconia?
For front teeth where tooth tissue is sufficient and bite load is low, porcelain laminate veneer is preferred for both less tooth preparation and highest light transmittance. Zirconia is the more appropriate choice for back teeth or front teeth with large material loss. In some cases a combination is applied. The correct decision is determined by the dentist following clinical examination and digital analysis.
Can smokers have smile design?
Yes, however certain points need to be taken into consideration. Smoking increases the risk of gum disease, slows the healing process and negatively affects teeth whitening results. For this reason, checking gum health before starting treatment is mandatory. After treatment, reducing or quitting smoking is recommended to maintain whitening effectiveness.
Is Digital Smile Design (DSD) applied in every clinic?
No. DSD is an advanced planning method that requires specialist software, hardware and a trained team of dentists. At our clinic, DSD and visagism are applied together; the patient sees and approves the digital simulation before treatment begins. This approach is available at very few centers in the Ankara İncek area.
Can the gums also be changed in smile design?
Yes, gum levels, form and color can be rearranged with the applications called pink aesthetics. Gummy smile (excessive gum visibility), asymmetric gum lines and dark gum color are corrected by laser or surgical methods. Performing only tooth restoration without planning gum aesthetics can negatively affect the final result.
