Orthodontics is the dental specialty concerned with the diagnosis, prevention and treatment of dental and jaw disorders. Crowded teeth, malocclusion and jaw discrepancies cause not only aesthetic concerns but also functional problems such as difficulty chewing, speaking and brushing. Suitable solutions for every age group are possible with metal brackets, porcelain brackets, clear aligner systems and early diagnosis functional appliances. As Sevgi Savaş Dental Clinic in Ankara İncek, we provide comprehensive orthodontic examination and personalized treatment planning.

What Is Orthodontics?

Orthodontics is the specialty that examines the position and relationship of teeth, dental arches and the jaw, and corrects disorders in these regions with fixed or removable appliances. The goal of treatment is not merely to align teeth; it is to achieve both functional occlusion (correct bite) and an aesthetic, healthy smile together.

Orthodontic treatment can be applied to children, adolescents and adults. Today, approximately half of the patients treated at orthodontic clinics are adults. Age is no barrier to treatment; orthodontic correction is possible at any stage where bone health is sufficient.

Dental and Jaw Problems Addressed by Orthodontics

Any of the following problems are among the conditions requiring orthodontic assessment. Early diagnosis shortens treatment duration and increases the permanence of results.

Crowded Teeth

Teeth are overlapping or rotated; cleaning is difficult, risk of decay has increased.

Upper Protrusion (Protruding Teeth)

Significant forward positioning of upper front teeth relative to lower teeth.

Deep Bite (Overbite)

When biting, the lower front teeth are completely covered by the upper teeth.

Open Bite

When back teeth contact, a gap remains between front teeth; swallowing becomes difficult.

Crossbite

Lower teeth sit outside the upper teeth; can lead to jaw asymmetry.

Diastema (Gap)

Aesthetic and functional gaps between teeth, especially visible between the upper front teeth.

Midline Discrepancy

Misalignment of the centrelines of upper and lower teeth; affects facial symmetry.

Narrow Jaw

Due to the narrow structure of the upper or lower jaw, insufficient space remains for the teeth.

Orthodontics with Metal Brackets (Classic Braces)

Metal brackets are the most established and proven method of orthodontic treatment. Small metal brackets bonded to the teeth and the wire running between them gradually guide the teeth to their ideal position.

Advantages of Metal Brackets Applicable to every age group and complexity level · Reliable solution in advanced cases where clear aligner is ineffective · Controllable and predictable tooth movement · Fixed system requiring no patient compliance · Generally the most economical orthodontic option
Who Is It Suitable For? Severe crowding, crossbite or open bite cases · Complex plans requiring jaw asymmetry correction · Child and adolescent patients (fixed system is advantageous for compliance) · Any situation where clear aligner is insufficient

Orthodontics with Porcelain (Clear) Brackets

Porcelain brackets work on the same mechanical principle as metal brackets; the only difference is the material and appearance. With their ceramic structure close to tooth colour, they attract much less attention than metal brackets. They are especially preferred by adult patients with high aesthetic concerns.

Clear Aligner Treatment

Clear aligners are removable and nearly invisible orthodontic appliances produced on a personalized basis based on a digital dental impression. Invisalign is the best-known brand of this system; similar technologies from different manufacturers are also available.

Each set of aligners moves the teeth approximately 0.25–0.5 mm; they are generally changed for a new set every two weeks. They must be worn for at least 20–22 hours per day; they are removed during eating, drinking and tooth brushing.

Who Is It Suitable For?

Mild and moderate crowding, gaps, mild malocclusion. For complex jaw problems or advanced malocclusion, the bracket system is more effective.

Aesthetic Advantage

The treatment goes unnoticed from outside in professional life, social settings and special events. It is the great preference of adult patients.

Hygiene and Comfort

Being removable makes tooth brushing easier and preserves gum health. No irritation caused by metal brackets is experienced.

Patient Compliance Is Critical

It must be worn for 20–22 hours per day. Removing or forgetting the aligners frequently extends treatment time and negatively affects results.

Not Every Case Is Suitable for Clear AlignerClear aligner is successfully applied in cases up to moderate severity. For advanced crowding, serious skeletal problems or cases requiring jaw surgery, a bracket system or combination treatment may be more appropriate. Suitability is determined by clinical examination.

Early Diagnosis and Orthodontics in Children

Early detection of orthodontic problems significantly reduces treatment duration and complexity. Interventions made while the jawbone is in the growth phase can guide development and prevent severe cases in the future.

According to international standards, the first orthodontic assessment should be performed at age 6–7. At this age, correction of all teeth is not expected; rather the aim is to detect problems affecting jaw development and intervene at the right time.

Conditions Requiring Early Intervention Crossbite (before unilateral jaw asymmetry develops) · Narrow jaw (can be corrected with expansion appliance before growth is complete) · Upper protrusion (injury risk and aesthetic concern) · Habit-related problems (thumb sucking, tongue thrusting)
Benefits of Early Treatment Space is created for permanent teeth · Longer and more complex treatments in the future are prevented · The child's social and psychological development is supported · In some cases prevents future tooth extractions

Functional Orthodontics and Correct Swallowing Therapy

Functional orthodontics addresses not only the alignment of teeth but also the balance of jaw muscles, the correct swallowing reflex and oral habits. Incorrect swallowing (such as forward tongue thrusting), thumb sucking or mouth breathing habits can adversely alter tooth positions and jaw development.

With functional appliances and myofunctional exercises, these habits are corrected, jaw muscles are balanced and permanent teeth settle into healthier positions. This treatment provides the highest efficiency especially in the 6–12 age range; skeletal correction is not possible after bone development is complete.

Mouth Breathing Is an Orthodontic Risk FactorChronic mouth breathing creates a basis for long face development, narrow upper jaw and malocclusion. If your child sleeps with their mouth open at night, it is recommended that this be evaluated together with an ENT specialist.

Adult Orthodontics

Contrary to widespread belief, orthodontics is not exclusive to children and adolescents. Today, approximately half of orthodontic patients are adults. In adults, bone development is complete; therefore only tooth movement is possible — skeletal guidance cannot be performed. However, this does not mean treatment is ineffective.

Pre-Implant Preparation

If neighboring teeth have tilted into a gap due to tooth loss, orthodontic correction is required before placing an implant or bridge.

Combination with Periodontal Treatment

Teeth that have become spaced due to gum disease can be brought closer together with orthodontic treatment; the retention wire functions as a splint.

Aesthetic Concern

Adults who are uncomfortable with their smile, particularly those active in professional life, can undergo a comfortable treatment process with porcelain brackets or clear aligner.

Temporomandibular Joint (TMJ) Relief

Incorrect bite can cause clicking, pain and headaches in the jaw joint. Orthodontic correction can reduce these symptoms.

Hygiene and Caries Prevention

Aligned teeth are easier to brush; spaces can be cleaned. Crowding is an important risk factor for decay and gum disease.

Confidence and Quality of Life

Research shows that orthodontic treatment has a positive effect on self-confidence and social life in adults.

Retention Phase – The Most Important Stage of Treatment

The retention phase is the critical process following the completion of active orthodontic treatment in which the teeth are stabilized in their new positions. When brackets are removed or aligner treatment ends, teeth tend to return to their previous positions under the influence of surrounding soft tissues. If this phase is neglected, treatment results can deteriorate over time.

Fixed Retainer (Bonded Retainer) A thin wire is bonded to the back surface of the teeth · Requires no daily use; it is permanent · The most reliable long-term protection for front teeth · Flossing must be learned (thread is passed under the wire) · Check-up at least once a year is recommended
Removable Retainer Plate Transparent, thin plate; worn at night · Supports or serves as an alternative to the fixed wire · Easier to clean; hygiene advantage · Duration and frequency of use determined by dentist's recommendation · Retention cannot be maintained if lost or neglected
Retention Is LifelongTeeth continue to move when independent of the retainer appliance. Stopping retention by thinking "treatment is over" can nullify the effects of years of orthodontic treatment. Use of the retainer appliance can last a lifetime according to the dentist's recommendation.

Orthodontic Treatment Process

1
Comprehensive Orthodontic Examination

Intraoral and extraoral photographs, panoramic X-ray and cephalometric analysis are used to evaluate the dental, jaw and facial structure holistically.

2
Digital Impression and Treatment Planning

A digital oral impression (or conventional plaster impression) is taken. Which method will be used, the treatment duration and expected results are shared with the patient.

3
Active Treatment (Brackets or Aligners)

Brackets are applied or the first aligner set is delivered. Wire activation or aligner change is performed at check-up appointments every 4–8 weeks.

4
Detail and Final Adjustments

As teeth approach the target position, fine detail corrections are made; occlusion is optimized.

5
Bracket Removal / Aligner Completion

Active treatment is completed. Teeth are cleaned, result is confirmed with X-ray.

6
Retention

Fixed retainer and/or removable plate is applied. Retention instructions are explained in detail; regular check-ups are planned.

Orthodontics – Frequently Asked Questions

How many months does orthodontic treatment take to complete?
Treatment duration depends on the complexity of the problem and the method selected. Mild cases may take 6–12 months, moderate cases 12–18 months, and complex cases 18–24 months or longer. Clear aligner treatment may be shorter than the bracket system for mild cases. At our clinic, the estimated duration and plan are shared following the initial examination.
Are braces or clear aligners better?
Both are effective; the right choice depends on the case. Clear aligner is ideal for mild-moderate cases, aesthetics-conscious adults and those with hygiene sensitivity. Metal or porcelain brackets represent more reliable control for complex crowding, malocclusion and cases requiring jaw asymmetry correction. The best decision is made following examination.
Is orthodontics possible for adults?
Yes. After bone development is complete, skeletal guidance cannot be performed; however tooth movement is possible at any age. Today, approximately half of orthodontic patients are adults. Porcelain brackets or clear aligner offer a comfortable treatment process for aesthetics-conscious adults.
At what age should orthodontic assessment be performed for children?
The first orthodontic assessment is recommended at age 6–7. At this age, correction of all teeth is not expected; the aim is to detect problems affecting jaw development early. Intervention between ages 8–10 for problems such as crossbite and narrow jaw can prevent longer treatments in the future.
How many hours a day should clear aligners be worn?
They must be worn for at least 20–22 hours per day. They are only removed during eating, hot beverages and tooth brushing. Adhering to this rule directly determines treatment duration and the quality of results. Frequently removing or neglecting aligners extends treatment time.
How long should retention be used?
Retention can theoretically last a lifetime. The fixed retainer wire is left permanently in place; removable plates are used every night initially, then a few nights per week over time. Stopping retention significantly increases the risk of teeth returning to their previous position.
Is orthodontic treatment painful?
Mild pain and sensitivity for 1–3 days after wire activation or a new aligner change is normal. It is controlled with over-the-counter painkillers. Over time teeth get used to the pressure and discomfort decreases. No pain is felt during bracket bonding; there may only be a feeling of pressure.
How is tooth brushing done during orthodontic treatment?
A special V-cut orthodontic brush and an interdental brush are recommended for those using brackets. Brushing after every meal significantly reduces the risk of gum inflammation and decay. Clear aligner users can remove the aligners for normal brushing; the aligners should also be cleaned with a soft brush and cold water.
What is functional orthodontics and how does it differ from bracket treatment?
Functional orthodontics includes appliances that guide jaw development and correct oral habits and swallowing reflex. Bracket treatment moves existing teeth to their ideal positions. Functional treatment is applied in the 6–12 age range; it offers the opportunity to guide bone growth. Planning is usually done in two phases: phase 1 (functional) and phase 2 (bracket/aligner).
Is tooth extraction necessary during orthodontic treatment?
Not in every case. In severe crowding or if the mouth is too small, extraction may be planned to create space for teeth. However, in modern orthodontics it is possible to create space without extraction using methods such as jaw expansion and mini screws (TADs). The extraction decision is made with cephalometric analysis and comprehensive assessment.
What should I do if a bracket breaks or a clear aligner is lost?
If a bracket breaks, call the clinic within normal working hours if there is no pain; our phone number is in your contacts for emergencies. If a clear aligner is lost, wear the previous aligner and book an appointment as soon as possible. In both situations, rapid intervention is important to avoid deviating from the planned course of treatment.
