Holistic dentistry (biological dentistry) is a comprehensive approach that treats oral health not as independent from the rest of the body but as an inseparable part of it. Are the materials used biocompatible? Does the treatment support the body's natural balance? Has the connection between systemic diseases and oral health been taken into account? As Sevgi Savaş Dental Clinic in Ankara İncek, we offer current holistic dentistry applications including the SMART protocol, resin infiltration, myofascial therapy, dental mesotherapy and prebiotic support.

What Is Holistic Dentistry?

Holistic dentistry is a comprehensive dental approach that evaluates oral health in an integrated manner with general body health, prefers non-toxic and biocompatible materials, prioritizes minimally invasive methods and treats the patient's lifestyle and dietary habits as part of the treatment. It is also known as biological dentistry or complementary dentistry.

Safe Amalgam Removal with the SMART Protocol

Amalgam fillings consist of silver, tin, copper and approximately 50% mercury. They release mercury vapor in the oral environment as a result of chewing, temperature changes and corrosion. SMART (Safe Mercury Amalgam Removal Technique) is the safe removal protocol established by the IAOMT (International Academy of Oral Medicine and Toxicology) that protects the patient and dentist during amalgam removal.

At our clinic, amalgam removals are performed with equipment and methods compliant with the SMART protocol; following removal, the tooth is restored with biocompatible ceramic or composite material.

Rubber Dam Isolation

The working area is completely isolated with a rubber sheet that prevents amalgam particles from spreading inside the mouth.

High-Volume Vacuum Aspiration

Mercury vapor and amalgam particles are immediately suctioned with a special high-volume aspirator; inhalation through the respiratory tract is prevented.

Oxygen Support and Protective Equipment

The patient protects their breathing with an oxygen mask during removal. The dentist and assistant use special clothing, masks and equipment.

Room Air Filter

A room air purifier is kept active in the clinic; mercury vapor released into the environment is filtered. Preparation is made with activated charcoal rinse.

Amalgam Removed in Chunks

The filling is removed in large pieces rather than being ground into powder; this method significantly reduces the amount of mercury released.

Biocompatible Restoration

After removal, the tooth is restored with ceramic inlay/onlay, all-ceramic crown or composite filling. No metal is used.

Can Every Dentist in Every Clinic Perform Amalgam Removal?

No. Safe amalgam removal is a specialized application requiring IAOMT training and knowledge of the SMART protocol. Removal performed without appropriate equipment and protocol can result in much higher mercury exposure compared to a standard removal.

Resin Infiltration – Minimally Invasive Caries Treatment

Early-stage caries in tooth enamel or white/opaque lesions formed after trauma/fluoride traces could previously only be treated with abrasion using traditional methods. Resin infiltration is a modern minimally invasive method that treats these stains and initial caries without any abrasion of the tooth tissue.

How Is It Applied? The tooth surface is prepared with a special acid gel · Low-viscosity resin material penetrates the lesion · Polymerized and hardened with light · The procedure takes approximately 15–20 minutes · No anesthesia or abrasion required
In Which Cases Is It Applied? White spots caused by fluoride traces (fluorosis) · White demineralization formed under orthodontic brackets · Early-stage enamel caries (before cavitation) · Developmental enamel defects

Resin infiltration is one of the best practical examples of the "minimally invasive" principle of holistic dentistry: tooth tissue is preserved, aesthetics are improved and caries is halted.

Myofascial Therapy – Jaw and Muscle Treatment

In holistic dentistry, treatment covers not only the teeth but also the jaw muscles and surrounding tissues. Myofascial therapy relieves pain, tension and functional disorders through special manual techniques applied to the jaw, neck and facial muscles.

Myofascial treatment delivers highly effective results in bruxism (teeth clenching/grinding), temporomandibular joint disorders (TMJ) and chronic head-neck pain. The connection between stress-induced muscle tension and oral-jaw health constitutes one of the core areas of the holistic approach.

Application Areas Bruxism (night/daytime teeth clenching) · Temporomandibular joint (TMJ) pain and clicking · Chronic headache and migraine · Neck and shoulder tension · Restriction in mouth opening
Benefits of Treatment Reduction in muscle tension and pain · Improvement of jaw joint function · Improvement in sleep quality · Slowing of tooth surface wear · Pain control without medication dependence

Dental Mesotherapy

Dental mesotherapy is a complementary treatment method in which very low doses of vitamins, minerals, antioxidants and when necessary local drug mixtures are applied to the jaw and facial muscles with fine needles. It offers a holistic option for patients who wish to avoid systemic medication and want to minimize the risk of side effects.

Areas of Use Muscle relaxant support in bruxism (teeth clenching) treatment · Temporomandibular joint pain · Chronic muscle tension and spasm · Inflammation control in the jaw region
Advantages Minimal systemic absorption; low risk of side effects · Local effect; direct access to target tissue · Natural components that do not harm general health · Synergistic effect in combination with myofascial therapy
Myofascial Therapy + Dental Mesotherapy CombinationAt our clinic, these two methods can be planned together. While myofascial manual techniques soften muscle tissue, dental mesotherapy supports inflammation control and local healing. It is rare to see this combination offered as a separate application at competing clinic sites.

Systemic Diseases and Oral Health Connection

The mouth is not an isolated region from the rest of the body. Oral flora imbalances and chronic oral infections can create systemic effects in areas such as the heart, diabetes, respiration and pregnancy. Holistic dentistry places this connection at the centre of treatment planning.

Cardiovascular Diseases

Periodontal bacteria and inflammation are associated with heart valve infection, atherosclerosis and increased cardiovascular risk. Gum health is critical for heart patients.

Diabetes

Gum disease progresses faster in diabetic patients and becomes more difficult to control. On the other hand, chronic gum inflammation can negatively affect blood sugar control. A bidirectional relationship exists.

Pregnancy

There is evidence that untreated gum disease increases the risk of low birth weight and premature birth. Oral health monitoring during pregnancy is a priority.

Respiratory Diseases

The bacterial load in the mouth can reach the lungs through aspiration. Oral hygiene is an inseparable part of the treatment plan for patients at risk of COPD and pneumonia.

Osteoporosis

A decrease in bone mineral density can affect the jawbone and implant success. Some medications used in osteoporosis treatment should be evaluated before tooth extraction.

Stress and Bruxism

Elevated cortisol increases gum inflammation and suppresses immune response. Stress management is a complementary component of treatment in holistic dentistry.

For this reason, a personalized treatment plan is created at our clinic by evaluating each patient's general health status, medications and systemic diseases.

Oral Flora Protection: Prebiotics and Probiotics

The oral flora is a dynamic ecosystem like the gut flora. The balance between beneficial and harmful bacteria is a direct determinant of tooth decay, gum disease and bad breath. Antibiotic use, unhealthy nutrition and stress can disrupt this balance.

Benefits of Probiotic Support Suppresses the proliferation of harmful oral bacteria (S. mutans, P. gingivalis) · Reduces the risk of gum inflammations · Provides support for bad breath (halitosis) control · Restores flora balance after antibiotic treatment
Nutritional Recommendations in the Holistic Approach Reduction of sugar and refined carbohydrate consumption · Flora support with fermented foods (kefir, yoghurt) · K2 and D3 vitamins; critical for dental and bone health · High antioxidant nutrition; reducing inflammation

All-Ceramic Crowns – Metal-Free Restoration

The fundamental principle in holistic dentistry is that restorative materials must be biocompatible. The metal substructure in traditional metal-porcelain crowns can create a dark line at the gum, react with oral fluids and create a basis for allergic reactions.

All-ceramic crowns (E.Max, zirconia) completely eliminate these problems: there is no metal, biocompatibility is high, and aesthetics are superior with natural light transmittance. All-ceramic materials are also the first choice for restoration of remaining tooth tissue following amalgam filling removal.

E.Max (Lithium Disilicate) Excellent light transmittance in front teeth · Most natural-looking aesthetics closest to natural tooth · Metal-free, zero allergy risk · Ideal for front teeth and premolars
Zirconia Crowns Use on both front and back teeth · High fracture resistance · Does not create dark colour at the gum · First preference for patients with metal allergy

Holistic Dentistry – Frequently Asked Questions

What is holistic dentistry and how does it differ from conventional dentistry?
Holistic dentistry is an approach that evaluates oral health in an integrated manner with the rest of the body, prefers biocompatible materials and prioritizes minimally invasive methods. Its fundamental differences from conventional dentistry are: it does not use amalgam, it prioritizes biocompatibility in material selection, it incorporates the systemic disease connection into treatment, and it places emphasis on preventive applications that support the oral flora.
What is the SMART protocol — do I have to have my amalgam fillings removed?
SMART (Safe Mercury Amalgam Removal Technique) is the safe amalgam removal protocol established by the IAOMT. The decision to have your amalgam fillings removed should be based on a personal health assessment; it is not mandatory for every patient. However, if the removal decision has been made, having it done with the SMART protocol rather than standard removal significantly reduces mercury exposure. Removal without a protocol can lead to higher risk.
Which white spots does resin infiltration treat?
Resin infiltration treats white spots caused by fluoride traces (fluorosis), demineralization spots formed under orthodontic brackets and early-stage enamel caries (before cavitation forms). It is applied without any abrasion of the tooth tissue; this feature makes it one of the strongest examples of minimally invasive dentistry.
What is the difference between myofascial therapy and dental mesotherapy?
Myofascial therapy relieves tension and pain through manual techniques applied to the jaw and facial muscles. Dental mesotherapy applies very low doses of vitamins, minerals or drug mixtures to the muscle tissue with fine needles and supports local healing. The two methods are complementary to each other; they can be planned in combination at our clinic.
Is there a connection between oral health and heart disease?
Yes. Bacteria causing periodontal (gum) disease and chronic inflammation are associated with atherosclerosis and cardiovascular disease risks. Antibiotic prophylaxis may be required before dental procedures in patients with heart valve problems. In the holistic approach, this connection is incorporated into the treatment plan.
Are there metal-free implants?
Yes. Zirconia (ceramic) implants are metal-free biocompatible implants that are an alternative to titanium. They do not release metal ions, are electrically neutral and do not provoke the immune system. They are also aesthetically superior; no grey colour is reflected from the gum. They can be the first choice for patients with metal allergy or those who prioritize biological dentistry principles.
Are oral probiotics truly beneficial?
Research shows that certain probiotic strains suppress harmful bacteria in the oral flora and have a positive effect on gum inflammation and bad breath. In the holistic approach, probiotics are recommended as a side support for treatment; they do not replace oral care and professional cleaning.
Can a diabetic person benefit from holistic dentistry applications?
Yes, especially so. Gum disease progresses faster in diabetes and its treatment becomes more difficult. As control of oral inflammation in the holistic approach is directly connected with blood sugar stability, the treatment plan takes this equation into account. Nutrition, stress management and oral flora support are particularly important for diabetic patients.
What is the difference between an all-ceramic crown and a metal-porcelain crown?
In metal-porcelain crowns, the metal substructure can over time create a dark line at the gum and lead to the risk of oral galvanism. All-ceramic crowns (E.Max, zirconia) contain absolutely no metal; they offer a much more aesthetic appearance with natural light transmittance, their biocompatibility is high and there is no allergy risk. They are the standard option in holistic dentistry.
What is oral galvanism?
Oral galvanism is the formation of a galvanic electrical current by different metal-containing materials in the mouth in the saliva environment. This current can cause symptoms such as metallic taste sensation, headache, sleep disorder and tooth sensitivity. In the holistic approach, this risk is eliminated by avoiding metal-containing dental materials.
